"Form Publisher is a free add-on for Google Forms that will automatically generate a Google Doc based on a template you have created. The add-on is triggered when a new form is submitted. It enters the form responses into a doc and places that doc into a specified folder in your Drive.

Do you want to send a copy of that document to the person filling out the form? No problem. Form Publisher will do that too.
